CommuniCare aims to improve access to community services, health, and well-being by improving coordination and communication between service providers in Wyoming and by addressing clients’ Social Determinants of Health.
The Centers for Medicare and Medicaide Services developed the Accountable Health Communities Health-Related Social Needs Screening Tool. Growing evidence shows that if we deal with unmet health related social needs (HRSN) like homelessness, hunger, and exposure to violence, we can help undo their harm to health. Just like with clinical assessment tools, providers can use the results from the HRSN Screening Tool to inform patients’ treatment
plans and make referrals to community services.
Together, these tools help organizations streamline access to Wyoming's health and social services by connecting providers and addressing barriers to care, helping community members lead healthier, better lives.
