Re-establishing Team Based Care Workflows: Creativity that Leads to Sustainability

As the new normal is upon us, it is time to re-engage patients in their health journey.  Managing chronic conditions and completing preventative care are the foundational services for population health.  Improving population health takes a team.  The team-based approach to care is more than just getting along as a team, the key elements must be reviewed continually as we address current issues by implementing creative care models in our practices.   Embracing health promotion, using team-based care delivery models, and incorporating payment models to optimizing patient engagement creates the foundation of value-based care. It’s time to re-engage in this new normal and move forward.

2. How to identify team members and incorporate team-based care into the workflows across the continuum of care in and around TCM, CCM, and AWV
